Health, Safety & Environmental Bulletin No 130

                                           S Martin – 19 March 2009

                                             Near Miss – Site Crash

Please see enclosed self explanatory ExxonMobil Safety Alert that relates to one of our
own Xmo Strata sign maintenance teams.

Lea Handley and Phil Morris’ use of defensive parking, in line with the guidelines laid
out in our JSA 002 – “Site Set-Up” meant that whilst our van sustained impact damage
from a car that skidded on the icy forecourt, the sliding vehicle was buffered away from
the working area and nobody was hurt.

As indicated in the safety alert, a Last Minute Risk Assessment had been carried out
when the weather conditions changed and it began to snow, but this incident occurred
whilst the team were packing up their equipment.

This is an excellent example where following the JSA genuinely averted what could
have been a much more serious incident.

The events that led to the incident were totally out of our control, but the severity was
           mitigated by our team’s actions.

Our individual safety is not solely in our hands, we have to rely on those around us to steer us away
from danger and warn us of potential hazards. We need safety awareness to be instinctive and form an
integral part of our working lives; it is this, in turn, that may save the lives of others.
