Incident Types
Please see enclosed safety flash regarding incident types shared with us by ExxonMobil.
Although the names of the incident categories vary slightly across different companies, the principle of the
different categories are consistent.
Some Potential Incidents (Hazard Identification) are common on site and can be seen by some as not worth
reporting. However, we encourage all incident types to be reported so that they can be addressed and trends
identified. If we find that there are lots of seemingly minor PI’s occurring, these can be identified and we can
liaise with the customers / sites to create a policy to eliminate or reduce the risk.
Where possible we encourage crews to rectify the hazard at source, and in 2017 we have rectified 42% of all
incidents at the time of discovery, as further 7% of incidents the risk was controlled/reduced on site. If you see a
hazard on site always assess the possibility of rectifying it whilst you are there, however carry out a LMRA first
to ensure you are not putting yourself or others at risk by doing so.
If you are unsure about the incident categories, or are not sure how to categorise a specific incident, contact your
line manager.
We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
Ultimately, your safety is in your hands – but we will do all we can to keep you well informed!
