Health, Safety & Environmental Bulletin No 443

                                                 24 December 2014

                                             Electric Shock Incident

Please see enclosed safety alert regarding an electrical shock that was shared with us by Aecom.

This incident occurred due to incorrectly wired hand dryer that had been isolated and removed. The term
“borrowed neutral” used in the bulletin means that the neutral conductor of another circuit had been connected to
the hand dryer, effectively connecting the two circuits together at the hand dryer. So isolating (LOTO) the hand
dryer circuit did not prevent it from becoming live via the second circuit.
When disconnecting signage, always test the circuit to ensure the cables are dead before touching them, and
beware that even if you have correctly isolated the conductors can be (or become) live. Protect the cable ends by
temporarily securing them in connector blocks – don’t leave bare conductors open to be touched.

We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
Ultimately, your safety is in your hands – but we will do all we can to keep you well informed!
