                                                        Fatality



           Please see enclosed a  bulletin  regarding  a fatality in South Africa. This bulletin has been responsibly
           shared by Jacobs and relates to an engineer working in a forecourt chamber.

           Although we don’t work in manholes, this incident highlights the serious consequences of working near
           any area where explosive vapours may accumulate.

           Always  check  the  site  hazardous  area  plan  before  starting  any  work  that  could  generate  an  ignition
           source,  remember  the  hazardous  zones  in  the  blue  book  are  only  suggested  layouts  –  site  specific
           layouts may include larger areas.

           Do not use any electrical equipment in the hazardous area unless it is  specifically designed for use in
           hazardous zones.  If you have any doubts or concerns contact your line manager immediately.
