                                                 Manhole Fatality



           Please see enclosed a self-explanatory Gilbarco bulletin that relates to a forecourt fatality.

           Although we don’t work in manholes, this incident highlights the serious consequences of working near
           any area where petrol vapours may accumulate.

           Always  check  the  site  hazardous  area  plan  before  starting  any  work  that  could  generate  an  ignition
           source,  remember  the  hazardous  zones  in  the  blue  book  are  only  suggested  layouts  –  site  specific
           layouts may include larger areas.

           If you are working in a hazardous area, specific controls will be needed to ensure you do not generate
           an  ignition  source  –  make sure you understand the JSA/JHA/SMS before starting work.  If you  have
           any doubts or concerns contact your line manager immediately.
