Although the names of the incident categories vary slightly across different companies, the principle of the different categories are consistent.
Some Potential Incidents (Hazard Identification) are common on site and can be seen by some as not worth reporting. However, we encourage all incident types to be reported so that they can be addressed and trends identified. If we find that there are lots of seemingly minor PI’s occurring, these can be identified and we can liaise with the customers / sites to create a policy to eliminate or reduce the risk.
Where possible we encourage crews to rectify the hazard at source, and in 2017 we have rectified 42% of all incidents at the time of discovery, as further 7% of incidents the risk was controlled/reduced on site. If you see a hazard on site always assess the possibility of rectifying it whilst you are there, however carry out a LMRA first to ensure you are not putting yourself or others at risk by doing so.
